We provide bespoke domiciliary care and support packages for 18 adults, in 14 properties, all with learning disabilities, Autism and complex needs.
Supported living services for individuals with a Learning Disability. Clients who live in their own home within the Supported Living Service can choose our services from a few hours a week, or 24/7 these services cover support, personal care and life skills.
Our service ensures that Clients feel safe and secure in their own home and within the community, so that they can enjoy an independent active life.
We follow a person centered approach to care and support; empowering the client to take the lead role and participate as fully as possible in all decision making processes.
Our care packages cover a range of services from direct care, support with domestic living, supporting service users to undertake activities within the community, and an outreach day service taking individuals out of their family home.
Depending on the assessed needs of each person, this will generate an individual plan of care designed to achieve the very best outcomes for our service-users.
